To Whom It May Concern,

Pursuant to the Freedom of Information Act (5 U.S.C. § 552) and the Privacy Act (5 U.S.C. § 552a), I respectfully request copies of the following documents related to my incarceration:

1. My full Inmate Central File (ICF), including all classification records.
    

2. My SENTRY report, including custody classification, risk assessment scores, program participation, any Public Safety Factors (PSFs), and any codes or notations associated with “Walsh Act” restrictions.
    

3. Any administrative notes or justification memoranda explaining or recommending restrictions on communication (such as email bans) during my incarceration.
    

I am specifically seeking any and all documents used to justify any “Walsh Act”-based restrictions, or classification codes and descriptions that contributed to the restriction.

Please include all responsive records, even if they are considered internal memoranda, emails, or administrative determinations.

If any portion of this request is denied, please specify the legal basis for the denial and segregate and release any non-exempt portions of the documents. If possible, I request that these records be provided electronically.
